This is first rip of this skin for 4.0.X version so feel free to report BUGs in this thread, im not genius so wait and everything u find will be fixed in next update.
Works with CMS, Forum and Blog, also im not sure if i included all images but dont worry i have them.
Unfortunately i did not create PSD of that Logo but you can replace it with any other PNG transparent Logo.
